Death Procedures

What do you do when there is death *touch wood* among your family members, relatives, or friends?  To better prepare ourselves, I have compiled a list of procedures below for various regions in Malaysia to facilitate quick references.  Please note this is a work-in-progress and will update this post when I stumble upon more information.

Peninsular Malaysia

Natural Causes

REGISTRATION OF DEATH BY NATURAL CAUSES

Application for registration of death by natural causes occurring in Malaysia and registered within 7 days from the date of death.

APPLICATION PROCEDURES

Applicants need to complete Form JPN.LM02 and attach the supporting documents. Registration of death can be made at any NRD Office throughout Peninsular Malaysia.

Applicants only need to complete the new Death Registration/Burial Permit JPN.LM02 (Pin.1/11) form and attach the supporting documents.  Registration of death can be made at any Police Station, Hospital, or Dewan Bandaraya Kuala Lumpur (DBKL).  As soon as application is submitted, a validated copy will be provided as Burial Permit for burial arrangements, and automatically the application will be registered at NRD and other government agencies (i.e. SPR, etc).  Applicants only need to visit NRD to collect the Death Certificate (JPN LM03).

SUPPORTING DOCUMENTS

  1. Original Identity Card or personal identification of the deceased
  2. Identity Card or personal identification of the informant
  3. Proof of death documents such as burial permit
  4. Certification by Medical Practitioner as to the cause of death [Form JPN.LM09 or JPN.LM10 (Post Mortem)]

FEES

  • No application fee.

Late Registration

LATE REGISTRATION OF DEATH

Late registration of death application is for death occurring in Malaysia and registered after 14 days from the date of death. However, body that is not found CANNOT BE REGISTERED.

[Section 21, Act 299]

APPLICATION PROCEDURES

Applicants need to complete Forms JPN.LM02 and JPN.LM28 and attach the supporting documents. Registration of death can be made at any NRD Office throughout Peninsular Malaysia. Application is accepted after the result of the search is obtained (Form JPN.LM14) except if the death occurs after 2003. However, body that is not found CANNOT BE REGISTERED.

SUPPORTING DOCUMENTS

  1. Personal identification document of the deceased (if any)
  2. Personal identification document of the informant (if non-citizen)
  3. Certificate of Death JPN.LM09/JPN.LM10 (If death occurs in a hospital)
  4. Burial Permit

FEES

  • Processing fee :- RM 10.00
  • Compound not exceeding RM 50.00 if application is approved.

 

Delayed Registration

DELAYED REGISTRATION OF DEATH

Delayed registration of death application is for death occurring in Malaysia and registered within 8 to 14 days from the date of death. However, body that is not found CANNOT BE REGISTERED.

[Section 19, Act 299]

APPLICATION PROCEDURES

Applicants need to complete Form JPN.LM02 and attach the supporting documents. Registration of death can be made at any NRD Office throughout Peninsular Malaysia.

SUPPORTING DOCUMENTS

  1. Certificate of Death JPN.LM09/JPN.LM10 (if death occurs in a hospital) or Burial Permit
  2. Personal identification document of informant (non-citizen)
  3. Personal identification of the deceased (if any)
  4. Form JPN.LM25 / JPN.LM26 if identity card/Certificate or Extract of Birth Register of the deceased is not submitted.

FEES

  • Processing fee: RM 5.00

 

While On Pilgrimage

REGISTRATION OF DEATH WHILE ON PILGRIMAGE

Every death occurring while on pilgrimage should be reported to the registrar in Hedjaz (an NRD official on duty in Mecca and Medina) within 40 days from the date of death during the Pilgrimage Season; Or report to the NRD Headquarters counter within 6 months from the date of death.

[Section 4(1)(a) and (b), Act 152]

APPLICATION PROCEDURES

Hedjaz death registration application can be made at any NRD Office throughout Peninsular Malaysia by submitting Form JPN.LMH2 and attaching the supporting documents. However, approval is only made at NRD Headquarters.

SUPPORTING DOCUMENTS

  1. Documentary proof of death – Letter of Confirmation of Death from the Saudi Arabia Government or burial permit
  2. Personal identification documents of the deceased – Certified copy of passport, identity card, birth certificate (if any)
  3. Personal Identification Document of the Informant (if informant is not the parents)
  4. Other supporting documents such as flight ticket (if any)

FEES

  • No fee is imposed.

 

Correcting Error

CORRECTING ERROR OF INFORMATION IN CERTIFICATE OF DEATH

Clerical Error

APPLICATION TO CORRECT CLERICAL ERROR

Application to correct minor error which only involves spelling error which if corrected does not alter the fact in the Certificate of Death.

[Section 27(2), Act 299]

APPLICATION PROCEDURES

Applicants need to complete Forms JPN.LM21 and JPN.LM22 and attach the supporting documents. Registration of death can be made at any NRD Office throughout Peninsular Malaysia.

SUPPORTING DOCUMENTS

  1. Personal identification documents of parents (if non-citizen)
  2. Personal identification document of guardian (if non-citizen) proven by Letter of Permission of Guardianship from JKM
  3. Letter of authorization from natural parents who give permission to guardian to make alteration application
  4. Personal identification document of applicant (if non-citizen) who exceeds 21 years old
  5. Personal identification document of the next-of-kin of the deceased/applicant (if non-citizen)
  6. Other documents proving errors

FEES

  • RM 2.00 on submission of application.
  • RM 5.00 for print-out of extract of Death Certificate.

 

Error Of Fact

APPLICATION TO CORRECT ERROR OF FACT

Application to correct error of fact where the information other than the name of the deceased provided by the informant during registration of the death is incorrect or not available. The applicant needs to submit supporting documents to prove the error of fact.

[Section 27(3), Act 299]

APPLICATION PROCEDURES

Applicants need to complete Form JPN.LM21, JPN.LM22, Statutory Declaration and attach supporting documents.

SUPPORTING DOCUMENTS

  1. Personal identification documents of parents (if non-citizen)
  2. Personal identification document of guardian (if non-citizen) proven by Letter of Permission of Guardianship from JKM
  3. Letter of authorization from natural parents who give permission to guardian to make alteration application
  4. Personal identification document of applicant (if non-citizen) who exceeds 21 years old
  5. Personal identification document of the next-of-kin of the deceased/applicant (if non-citizen)
  6. Other documents proving errors

FEES

  • RM 2.00 on submission of application.
  • RM 5.00 after application has been processed and approved.
  • RM 5.00 for print-out of extract of Death Certificate.

 

Extract Certificate

EXTRACT OF DEATH CERTIFICATE

Application to obtain copy of death register by qualified applicant on payment of the prescribed fees.

[Section 32, Act 299]

APPLICATION PROCEDURES

Applicants need to complete Form JPN.LM13 and attach the supporting documents. Application can be made at any NRD Office throughout Peninsular Malaysia.

SUPPORTING DOCUMENTS

  1. Personal identification document or valid travel document (if non-citizen)
  2. Departmental Official Letter to apply for a search of the register (for application from Government Department)
  3. Any document or information that can be used as a basis to make a search of the death register such as Burial Permit/Police Report.

FEES

  • Application fee : RM 5.00

 

Extract Certificate In Hedjaz

EXTRACT OF DEATH CERTIFICATE IN HEDJAZ

Application to obtain copy of death register by qualified applicant on payment of the prescribed fees.

[Section 9(1), Act 152]

APPLICATION PROCEDURES

Applicants need to complete Form JPN.LM14 and attach the supporting documents. Application can be made at any NRD Office throughout Peninsular Malaysia.

SUPPORTING DOCUMENTS

  1. Copy of Death Certificate/ Report (if any)
  2. Original Personal Identification Document of the Deceased (if any)
  3. Form JPN.LM26
  4. Original or certified copy of Personal Identification Document or valid travel document (non-citizen)

FEES

  • Application fee: RM 2.00

 

Outside The Country

EXTRACT OF DEATH CERTIFICATE OUTSIDE THE COUNTRY

Application to obtain copy of death register by qualified applicant on payment of the prescribed fees.

[Section 9(1), Act 152]

APPLICATION PROCEDURES

Applicants need to complete Form JPN.LM02 and attach the supporting documents. Application can be made at any NRD Office throughout Peninsular Malaysia.  Note: Letter of Confirmation of Death will be issued to applicants.

SUPPORTING DOCUMENTS

  1. Original Certificate of Death issued by the country concerned that has been translated into Malay/ English
  2. Personal Identification Document of the deceased
  3. Certificate of Birth of the deceased (if any)
  4. Personal Identification Document of the applicant
  5. Certificate of death by High Commission/ Embassy/ Consul Office of Malaysia in the country concerned.

FEES

  • No fee is imposed.

 

Sabah

Normal Registration

NORMAL REGISTRATION OF DEATH

Applicants who register a death within 24 hours from the date of the death and the informant must be the mother/father/relative/person who knew of the death. Application can be made in Peninsular Malaysia and will be forwarded to Sabah for the approval of the Regional Registrar.

[Section 14(1), Sabah Ordinance, Cap. 123]

APPLICATION PROCEDURES

Applicants need to complete Forms B and N2 (Extract) and attach the supporting documents.

SUPPORTING DOCUMENTS

  1. Police Report for death which occur in a house
  2. Certificate of death for death which occur in a hospital
  3. Personal Identification Document of informant or valid travel document (if mother/father/informant is not a citizen)
  4. Police Report if identity card/certificate or extract of the Birth Register of the deceased is not submitted

FEES

  • No fee is imposed.

 

Late Registration

Applicants need to complete Forms B and N2 (Extract) and attach the supporting documents.

  3. Burial Permit
  4. Police Report for death which occur in a house
  5. Statement of Native Chief/Village Head and Witness Declaration

Delayed Registration

DELAYED REGISTRATION OF DEATH

Applicants who register a death within 24 hours to 10 days from the date of the death and the informant must be the mother/father/relative/person who knew of the death. Applications can be made in Peninsular Malaysia and will be forwarded to Sabah for the approval of the Regional Registrar.

[Section 16, Sabah Ordinance, Cap. 123]

APPLICATION PROCEDURES

Applicants need to complete Forms B and N2 (Extract) and attach the supporting documents.

SUPPORTING DOCUMENTS

  1. Police Report for death which occur in a house
  2. Certificate of death for death which occur in a hospital
  3. Personal Identification Document of informant or valid travel document (if mother/father/informant is not a citizen)
  4. Police Report if identity card/certificate or extract of the Birth Register of the deceased is not submitted

FEES

  • Application Fee : RM 5.00

 

Correcting Error

APPLICATION TO CORRECT CLERICAL ERROR

Application to correct minor error which only involves spelling error which if corrected does not alter the fact in the Certificate of Death.

Applicants need to complete Forms PSB 4 and PSB 4A and attach the supporting documents. Applicants should be 2 next-of-kins of the deceased. Submission can be made at any NRD Office throughout Sabah.

  1. Personal identification documents of parents (if non-citizen)
  2. Personal identification of guardian (if non-citizen) proven by Letter of Permission of Guardianship from JKM
  3. Letter of authorization from natural parents who give permission to guardian to make alteration application
  4. Personal identification document of applicant (if non-citizen) who exceeds 21 years old
  5. Personal identification document of the next-of-kin of the deceased/applicant (if non-citizen)
  6. Other documents proving errors

FEES

  • RM 5.00 – Application fee
  • RM 10.00 – Approval

 

Extract Certificate

EXTRACT OF DEATH CERTIFICATE

Applicants need to complete Forms P and N2 and attach the supporting documents. Applications can be made in Peninsular Malaysia and will be forwarded to Sabah for the approval of the Regional Registrar.

SUPPORTING DOCUMENTS

  1. Personal identification document or valid travel document (if non-citizen)
  2. Departmental Official Letter to apply for a search of the register (for application from Government Department)
  3. Any document or information that can be used as a basis to make a search of the death register such as Burial Permit/Police Report.

Sarawak

APPLICATION PROCEDURES

Applicants need to complete Forms III and XI and attach the supporting documents.

SUPPORTING DOCUMENTS

  1. Confirmation of Community/Tribal Leader for Sarawak
  2. Police Report for death which occur in a house
  3. Form VIII (Medical Certificate of the Cause of Death)
  4. Form IX (After Post-Mortem)
  5. Personal Identification Document of informant or valid travel document (if mother/father/informant is not a citizen)
  6. Personal Identification Document of the deceased
  7. Police Report if identity card/certificate or extract of the Birth Register of the deceased is not submitted

AMENDMENT OF PARTICULAR OF DEATH

Clerical Error

APPLICATION TO CORRECT CLERICAL ERROR

The application must be to correct error in the death register which is categorized as clerical error. The applicant must be the mother/father/children/siblings/wife/husband. Applications can be made in Peninsular Malaysia and will be forwarded to Sarawak for the approval of the Regional Registrar

APPLICATION PROCEDURES

Applicants need to complete Forms XI and JPN.LM21 and attach the supporting documents.

Extract Certificate

EXTRACT OF DEATH CERTIFICATE

Extract of death certificate application for registry owner who has not been issued a death certificate/ application for replacement of death certificate that is missing/destroyed. The applicant must be the next-of-kin of the deceased/wife/husband/children/legal firm. Applications can be made in Peninsular Malaysia and will be forwarded to Sarawak for the approval of the Regional Registrar

[Section 26, Births & Deaths Registration Ordinance 1951 (Sarawak Cap. 10)]

APPLICATION PROCEDURES

Applicants need to complete Form XI and attach the supporting documents. Registration of death can be made at any NRD Office throughout Malaysia.

SUPPORTING DOCUMENTS

  1. Personal identification document or valid travel document (if non-citizen)
  2. Departmental Official Letter to apply for a search of the register (for application from Government Department)
  3. Any document or information that can be used as a basis to make a search of the death register such as Burial Permit/Police Report.
